2024 Rising Star Award Nomination Form

This young lady between the ages of 8 – 18 has goals in entrepreneurship. She is a role model to her peers, demonstrates strong leadership skills, academically successful and involved in volunteering in her community.

Nomination form and requested information must be submitted no later than April 30, 2024.
